In this area, you will find links to all the forms which are currently relevant to Beacons and other aspiring providers. Next to each link, there is a short explanation about who should use the form and when it should be used.

You can also access the forms by using links at various points throughout the website wherever you see a hyperlink.

This area will be updated should any of the forms be amended and all Beacons will be emailed with details of any changes.

 Beacon status

 
Beacon declaration of interest (DOI) template – to be used by those providers which have been invited to apply for Beacon status. This form will also be emailed out to such providers. For more information about Beacon status, please click here.
   
   

 Beacon innovation funding

   
Beacon innovation fund guidance notes and invitation to submit - will be emailed to all Beacons in advance of the funding round deadline date. To be used to submit a proposal for LSIS innovation funding.
   
Beacon innovation fund reporting template – will be emailed to each successful Beacon in receipt of innovation funding once signed contracts have been returned by the provider. Reporting dates will be determined by LSIS and populated in the form. The blank form here can be used if necessary. Please refer to individual contracts for reporting criteria.
   

Beacon innovation pilot write-up – to be completed one month after innovation project closure and submitted to the contact at the bottom of the form.
